The success of a dental implant and its ability to support a dental restoration is very much dependent upon how much bone is available in the site where the implant is placed.

There are a variety of factors that affect the bone volume including periodontal disease, trauma and infection. It is not unusual to open up a site in the mouth for implant placement and find that some of the critical supporting bone is missing. Replacing missing bone or adding to the existing bone is very often essential to the success of a dental implant and the ensuing restoration (supporting crowns).

We at Pacific Northwest Periodontics have the latest, state of art techniques available to replace missing bone. Using these techniques we can increase the height of bone, the width of bone and fill in all sorts of defects that develop when teeth are lost.

A SINUS LIFT is one of the procedures used to increase the availability of bone for maxillary (upper jaw) implants. The back part of the upper jaw is hollow and contains a space called the maxillary sinus cavity. The size of these cavities appears to be genetically pre-determined and varies from patient to patient. Sometimes the sinuses are enlarged and intrude on the areas where we want to place dental implants.

….If the back teeth in the jaw are not replaced in time after extractions, sinuses tend to enlarge (pneumatization), thereby resorbing available bone for the placement of implants at a later stage.

A dental implant cannot be placed in such areas as there would be no bone to hold it in place. Sinus lift or sinus augmentation is a method to increase the amount of bone in the pre-molar and molar area of the upper arch in order to prepare it for the insertion of one or more dental implants. It could be done in a single stage, incision-less manner (indirect sinus lift) or more elaborate multi-stage procedure (direct sinus lift), depending upon the amount of pre-existing bone and the increase in the height of bone that is required.


A thorough evaluation is provided at Pacific Northwest Periodontics before proceeding with any form of treatment. This includes XRAYS to study the anatomy of your jaws and sinuses. A CT (computed tomography) SCAN is also advised to accurately measure the height and width of your existing bone and evaluate the health of your sinuses.


In order to carry out a sinus lift, we use bone tissue originating from the patient's body (autogenous bone) or from a donor (human or bovine). At Pacific Northwest Periodontics we typically use xenograph (bovine bone in a bottle) which can stimulate the formation of new bone material.

During an indirect sinus lift, a small opening is made in the gum. A shallow hole is then prepared in the jaw bone and the floor of the sinus is elevated by using bone graft materials and specialized designed tools (osteotomes) to tap up the floor of the sinus. In most cases it is possible to place the implant right away. The implant is usually ready to be restored with a crown within 3-6 months. The main limitation to this approach is that there needs to be a reasonable amount of pre-existing bone present to stabilize the implant. There is also a limit to the amount of bone height that can be achieved.

In cases where the amount of remaining bone is very limited or soft, an incision is made in the gum to expose the jaw bone and an opening is then created into the sinus. Great care is taken to preserve the soft tissue lining (Schneiderian membrane) of the sinus as it is elevated upwards. The space underneath the membrane is filled with bone graft material which is then covered with a layer of collagen. Stitches (sutures) are used to close the surgical site. It is possible to achieve substantial bone augmentation with this approach. Usually implants can be placed into the area after a 6-9 healing period, during which time the bone graft matures.
